*Beloit Health System* at *Beloit Regional Hospice* is hiring for a *Social Worker- Hospice/Palliative Care* to join our team. In this position the Social Worker will play a crucial role in supporting patients with serious, life-limiting illnesses and their families. The responsibilities of this Social Worker will focus on emotional, social, and practical aspects of care to improve quality of life including: emotional and psychosocial support, care coordination and advocacy, advance care planning, family and caregiver support, resource connection, and financial assistance. This Social Worker may also need to assist with hospice patient coverage at times.

*Requires:*
* Completion of a Masters Degree in Social Work
* Current WI or IL Licensure, with ability to obtain the other (must apply within 30 days of hire)
* Hospice or Palliative Care experience is preferred
