Description

The Low Threshold Case Manager is responsible for the coordination and delivery of case management services to identified individuals at our Low Threshold Housing Program in that provides shelter, case management, housing advocacy and navigation and recovery referral services to individual identified as unsheltered/homeless with chronic substance use. Our program helps individuals address barriers to housing, support individuals who want to address/reduce their substance use and find permanent housing. The Low Threshold Case Manager reports to the Program Director for support and supervision.

Essential Functions:
  • Act in accordance with Victory Programs, Inc.'s Standards of Excellence & Philosophy Care at all times including maintaining professional ethics, boundaries and client confidentiality.
  • Provide supported navigation, weekly individual case management services, and appointment accompaniment for clients seeking housing, recovery, health or social service care linkage.
  • Participate in the development and facilitation of groups and workshops that address barriers to housing and other assessed needs.
  • Engage in relationship building, Nonviolent Crisis Intervention, Motivational Interviewing and conflict de-escalation as needed in the milieu space.
  • Maintain accurate and timely client records including assessments, case notes, referrals client center goals and discharge documentation.
  • Advocate on behalf of residents experiencing behavioral health, medical, benefits and legal barriers which prevent housing placement or stabilization.
  • Maintain a current working knowledge and database of referral sources such as detox, outpatient counseling, self-help groups and others to provide comprehensive resources to program residents.
  • Make appropriate referrals to local addiction treatment programs and monitor progress and consistency of treatment involvement.
  • Work in conjunction with the VPI Mobile Prevention Team to co-facilitate and coordinate all on-site housing, substance use, HIV/AIDS, STI related education, prevention and intervention groups.
  • Work closely with all members of the Envision team to develop and implement aftercare plans for housed clients.
  • Maintain accurate documentation and recording keeping in electronic health records, client case files and program logs.
  • Complete all internal and external reporting as needed.
  • Communicate effectively and maintain positive working relationships with all members of the program, agency team and network partners.
  • Enforce all policies and procedures related to maintaining a safe and appropriate residential environment; while promoting a healthy, supportive, inclusive community.
  • Attend supervision, staff meetings, and periodic professional development trainings and workshops as requested by supervisor.
  • Promote access to and participation in the community and neighborhood, including gathering and distributing information regarding community meetings, events, and resources.
Non-Essential Functions:
  • Participate and support the agency in program events, State House rallies, fundraising and advocacy events, rallies, walks and any additional functions.
  • Perform any other duty or task that may be requested from time to time by your supervisor or their designee.
Required Qualifications:
  • A minimum of 2 years' experience working in the field of substance use.
  • Experience working with the homeless population.
  • Working knowledge of HIV/AIDS and viral hepatitis, homelessness, substance use, STIs, and sex work as well as health-related social issues faced by people who use drugs.
  • Ability to work effectively with individuals of diverse race/ethnicity, sexual orientation, gender identity, sex and drug cultures, age groups, religion, country of origin, disability and economic status.
  • Excellent verbal communication skills including an ability to have open and affirming conversations about sex and substance use risk.
  • Ability to relate to residents in a helpful, directive, yet compassionate manner.
  • Proficient computer skills, including the use of Microsoft Word, and Excel.
  • Must be CPR certified (may be obtained upon hire)
  • Compete TB Screening (may be obtained upon hire)
  • Valid Massachusetts driver's license and safe driving points of 00, 98, 99, 01, 02, or 03.
Preferred Qualifications:
  • Bachelor's degree or two years of case management experience.
  • Experience working with poly-addicted persons.
  • Experience working in human services or harm reduction/prevention services.
  • Strong organizational, writing and communication skills.
  • Ability to work independently and cooperatively as a member of a professional team.
  • Three strong work related professional references; at least 2 from manager or supervisor.
  • Bilingual capacity.
Hours : Full-time position (40 hours/week) Monday - Friday 9am-5pm

Rate: $25.00/hour
