- 6 Months DSP Experience
- Valid Drver's License
Completion of Drivers Safety Course is mandatory upon hire

Physical Demands: •Ability to maintain focus and safety during extended driving periods. •Ability to lift, move and position adults or children in wheelchairs, vans, etc. •Frequent stooping, bending, kneeling and pushing and pulling of heavy objects such as furniture and wheelchairs.

Day-to-Day Responsibilities:See Attached
Major Job Duties and Responsibilities: •Drives City Access New York vehicle to transport participants to and from homes to community-based program settings. •Chaperones participants into the community on a daily basis and maintains close supervision of them. •Collects data for written program plans, i.e. daily notes, behavior plans, treatment plans. •Writes case notes and provides information on incidents as required. •Provide necessary program support/coverage. •Participates in community-based activities, to provide support and supervision of participants. •Provide physical and programmatic aid to participants where assigned. •Aids in daily operation of assigned program areas. •Assists with supervision of participants in assigned areas. •Initiatives precautions within established framework to assure that work is completed in a manner free from recognized hazards. •Notifies supervisor of any problems with the vehicle. •Participates in case conferences as needed. •Serves as member of interdisciplinary team. •Attends periodic supervisory conferences and required staff meetings. •Participates in in-service training programs. •Substitutes in other groups as may be needed. •Performs pre-trip inspection of vehicle in accordance with company policy. •Maintains vehicle log of mileage time and ensures that it is kept clean. •Performs other related duties as requested.
