The Clinical Performance Quality (CPQ) Pharmacy Operations team supports Optum's Care Delivery Organizations by delivering patient-centered, clinically collaborative medication consultations to help people live healthier lives. Our team focuses on the design, execution, and delivery of telephonic and digital engagement strategies designed to close Medicare star measure gaps in care, including medication adherence, comprehensive medication reviews for MTM completion, statins therapy in patient with diabetes and cardiovascular disease. Our interdisciplinary service delivery team is comprised of Pharmacists, Pharmacy Technicians, and Care Coordinators. Positions in this function consult with customers, members, patients, providers, internal departments and/or external third parties on any or all the following: The Clinical Pharmacist will perform telephonic, patient-centered clinical consults focused on Medication Therapy Management (MTM), Care for Older Adults annual comprehensive medication reviews, conduct outreach, and consult to support statin utilization in patients with diabetes and cardiovascular disease. In addition, the clinical pharmacist will field clinical concerns escalated by the medication adherence call center agents via warm transfer. The clinical pharmacist will be required to meet or exceed established productivity and quality metrics and work a flexible schedule to support the hours of operation of the business. You'll enjoy the flexibility to work remotely from anywhere within the U.S. as you take on some tough challenges. Primary Responsibilities: + Conduct telephonic medication consultations that meet/exceed production and quality metrics + Complete annual medication review of prescription and non-prescription medication with emphasis on patient assessment and counseling, safe and effective use of education, identification of potential drug interactions, and other related activities + Complete pharmacotherapy assessment of patients' claim, past medical history, and disease states + Assess patients for medication adherence barriers and develop a plan to maximize patients' medication success through improving or maintaining adherence to medications + Deliver high quality evidence-base To view full details and how to apply, please login or create a Job Seeker account
